Recurrent Carcinomas of the Thyroid



Abstract
characteristics of recurrences were studied in 45 patients with recurrent thgroid carcinomas during the period between 1981 and 1990, in pursuit of the improved operative treatment to lessen recurrence.
There were 9 males and 36 females with no differences in recurrent patterns. The frequent sites of recurrence in patients under 40 years of age were lgmphnodes in the anterior and lateral side of the neck, in contrast to the invasive lesion of
the
anterior neck and distant lesions in patients over 40 years of age. Twenty-two(59.5) of the 37 cases with papillary carcinoma showed recurrences in lymphodes of the anterior and lateral neck, and 5(62.5%) of the 8 cases with follicular carcinoma
showed
recurrences in the distant metastatic areas. There were 12nodal(60.0%) and 5 invasive local recurrences in lobectomy cases, and 8 nodal(36.4%) with 5 local recurrences in subtotal thyroidectomy cases. Most of the nodal recurrences occured with in
3
years after primary operations. Whereas, locally invasive recurrences were discovered during the period between 3 and 30 years after primary operations and recurrences in the distant metastatic lesions were after 10 years. Resectabilities in
secondary
operations were 68.9% in our study.
In conclusion, lymphnode clearance is more important than thyroidectomy per se at the time of primary operation to lessen recurrence because there are early nodal recurrences within 3 years.
